Full Name, Contact Email, Full Address, Purchase Location, Beer Brand, Beer Container (12oz Bottle, 12oz can, etc.), Packaging Date and Batch Number (this info can be found printed on the side of each case, on the shoulder of bottles, and on the underside of cans).
